When seeking a donation or sponsorship be sure to submit all forms at least 30 days in advance of your need.

Please note: All applications for 2023 will be reviewed and fulfilled by May 1, 2023.

How To Apply

Step 1. Forms:
Applicants must download and fill out our application form linked below as well as an IRS letter confirming 501(c)(3) tax exempt status (if applicable). Both forms will need to be saved and uploaded back to us. Applications without the necessary forms will not be eligible for fulfillment.
